Using Everest Forms – a WordPress form and quiz builder plugin, you can allow users to download their WordPress quiz form with responses and scores in PDF format. This feature is handy for educational websites, training platforms, and assessment forms where users need a record of their quiz performance.
Prerequisites:
- Active Installation of Everest Forms Pro v1.9.4 or above.
- PDF Form Submission addon (installed and activated).
- Survey, Polls, and Quiz addon (installed and activated).
Step-by-Step Configuration Guide #
After you’ve activated Everest Forms Pro and enabled the necessary addons, follow these steps to let users download their quiz form submission:
Step 1: Enable the “Download PDF after form submit” Option #
Navigate to your WordPress admin dashboard and go to Everest Forms > Settings. Now, click on the PDF Submission tab.

Scroll down to locate the Download PDF after form submit option. Once you get the option, toggle to enable this feature. Below, you’ll find Custom PDF Download Text, from which you can customize the download link text.

Click Save Changes to apply your settings.
Step 2: Create a New WordPress Quiz Form or Edit an Existing One #
Next, navigate to Everest Forms > Add New and create a new form. If you already have an existing quiz form, you can open it to edit it. From the form field panel, drag and drop the Checkbox field into your form builder. Now, add all your quizzes with the necessary instructions for each question.

Step 3: Enable Quiz and Configure PDF Submission #
Navigate to the Settings tab of the form. From here, click on Survey, Polls, and Quiz tab and toggle to Enable Quiz.

Next, click on PDF Submission tab and toggle to enable it on the form.

Now, scroll down to find the Download PDF after form submit button. Make sure it’s enabled. Also, you can customize the download link text from here.

Just below it, enable Show Quiz Scores if you want users to view their score in the PDF file. Save the form to make the changes.
Step 4: Embed WordPress Quiz Form #
Let’s embed the form on a page or a post so that users can access, fill out, and download their response.
Using the Everest Forms Embed button:
- Click on the Embed button on the form builder, and you’ll see a pop-up asking to select an existing page or create a new page.
- Based on your preference, perform the necessary action and click on Lets Go!

Using WordPress Shortcode:
- Copy the form shortcode from the form builder
- Paste it into any post or page:

Using Gutenberg Block:
- Add a new block in the WordPress editor
- Search for “Everest Forms”
- Select your quiz form from the dropdown

Step 5: Test for User Experience and PDF Generation #
Now, the workflow for the entire process will occur as follows:
- Form Submission: User answers quiz questions and clicks Submit.
- Results Processing: WordPress processes the quiz responses.
- PDF Download Option: A download link appears based on your configuration.
- Score Display: Scores are visible in the PDF if “Show Quiz Scores” is enabled.
- File Generation: A PDF is generated with quiz results and scores.
To test the download function for quiz submission, visit your page or post in an incognito/private window. Now, fill in the form and click on the Submit button.

A message with a download link for the PDF file for your submission should appear on your screen. Click on the link, and choose the location for the file to download on your device. You can now view the response in the file.
