The Survey Report provides a visual summary of all responses collected through your Everest Forms surveys. You can review results for every question, compare trends, switch between chart types, and export or print your findings all from your WordPress dashboard.
Prerequisites:
- Everest Forms Pro.
- Everest Forms - Survey, Polls and Quiz.
Access the Survey Report #
Go to Everest Forms → Entries in your WordPress admin. Select a form that contains survey fields, then click View Survey Report.

Review Summary Cards #
At the top of the report, you’ll see two summary cards:
- Total Responses: The number of people who submitted the survey, along with a trend indicator showing growth or decline compared to the previous period.
- Average Completion: The percentage of questions respondents typically complete before submitting.

View Question Cards #
Each survey question appears in its own card, listed in the same order as your form. Every card shows:
- A type tag (e.g., Scale Rating, Multiple Choice, Checkbox)
- The question title
- Answered and Skipped counts

Switch Between Chart Views #
For Multiple Choice, Scale Rating, and Star Rating questions, you can switch between four chart views using the toggle buttons on the card:
- List: Horizontal bars with percentages (default).
- Bar: Vertical bar chart.
- Line: Line graph with filled area.
- Pie: Pie chart with color-coded legend .

Question Types #
Radio #
Covers Multiple Choice, Checkbox, Radio, Select, and Dropdown fields. Each option shows a label, a progress bar, and a percentage. The top option is highlighted.

Scale Rating #
Displays a score gauge with three color-coded zones red (low scores), orange (mid-range), and green (high scores). Below the gauge, you’ll see a breakdown of each segment with its percentage and count. Four metric boxes appear above the chart: Average, Median, Most Chosen, and Top Box (percentage of respondents who chose the highest values).

Star Rating #
Shows the average rating with star icons on the left and horizontal bars for each rating level on the right.

Likert Scale #
Displays a table where each row is a statement and each column is a scale point (e.g., Strongly Disagree to Strongly Agree). Cells show response counts.

Yes / No #
Two side-by-side donut charts green for “Yes” and red for “No” each with a response count.

Text Responses #
Individual written responses are listed with their submission date. Long responses are truncated with a “…” button to expand. The first 5 responses are shown by default click Show More to see the rest.

Export the Report #
CSV Export #
Click Export in the header to download the full report as a CSV file. The file includes summary data, each question’s results with counts and percentages, and is named survey-report-[form-id].csv.

Save as JPG #
Click the three-dot menu (⋮) on any question card and select Save as JPG to download that card as a high-resolution image.

Print the Report #
Full Report #
Click Print in the header to open your browser’s print dialog. The printout includes the form name, summary metrics, and all visible question cards.

Single Question #
Click the three-dot menu (⋮) on a question card and select Print Question to print just that one card.
